423 DECLARATIONS OF DISCLOSABLE PECUNIARY INTERESTS AND PERSONAL OR PERSONAL AND PREJUDICIAL INTERESTS AND DECLARATIONS OF WHIPPING ARRANGEMENTS (IF ANY) - Cllr O'Sullivan declared a personal interest due to her position as an RDaSH Governor, and Cllr Ogg declared a personal interest, as he knew Mr Mitchell (item 426 refers).
No whip was declared.

425 MINUTES - Resolved - That the minutes of the proceedings of this panel held on 26 June 2017 and 17 July 2017, having been printed and circulated amongst the members, be taken as read and correctly recorded and signed by the chairman.
426 SUPPORTING THOSE WITH CO-EXISTING MENTAL HEALTH AND SUBSTANCE MISUSE ISSUES - The Chairman welcomed Susan Twemlow, Service Manager Health Improvement Team, Stewart Atkinson, Joint Commissioning Manager, Substance Misuse, Charlotte Spain, Specialist Social Worker, Jane Ellerton, Head of Strategic Commissioning, North Lincolnshire CCG, Jim Mitchell, Clinical Lead, RDaSH, and Julie Napolitano, Programme Manager, Transformational Programme Manager RDaSH, to the meeting. The Chairman explained that the witnesses had had been invited to update the panel on progress on implementing the scrutiny panel's recommendations on improving services for those with co-existing mental health and substance misuse issues.
The witnesses gave a short joint presentation, describing progress on each of the recommendations, and outlining related ongoing and completed workstreams. Members asked questions throughout, on issues such as implementation of the pathway, links to homelessness, the use of multi-disciplinary teams, discharge rates, and the role of Accident & Emergency.
Resolved - (a) That Susan Twemlow, Stewart Atkinson, Jane Ellerton, Charlotte Spain, Jim Mitchell and Julie Napolitano be thanked for their attendance and for providing an update, and (b) that Trust representatives be invited to a future meeting in the spring in order to ensure that the panel be kept informed of progress.
427 EAST MIDLANDS AMBULANCE SERVICE - QUALITY, PERFORMANCE AND SAFEGUARDING ISSUES - The Chairman welcomed Kirsty Morgan, Senior Quality Manager, to the meeting. It was explained that Kirsty had been invited to update the panel on the number and nature of Serious Incidents in recent years, and mechanisms to deal with these. Kirsty explained how Serious Incidents were handled, the guidance and legislation related to these, and comparative numbers in recent years. Members asked questions on issues such as triage systems, first responders, and staff reporting rates.
Resolved - (a) That Kirsty Morgan be thanked for her attendance and for providing an update, (b) that requested information be forwarded to the panel, and (c) that the situation be noted.
