Statement of Purpose - Home First Short Stay and Home First Community

Minutes:

1        STATEMENT OF PURPOSE – HOME FIRST SHORT STAY AND HOME FIRST COMMUNITY - The Director: Adults and Community Wellbeing submitted a report informing the Cabinet Member that the Home First Short Stay and Home First Community Statements of Purpose had both been reviewed and submitted to the Care Quality Commission and seeking approval to publish both documents on the council’s website.

 

The council was required to produce a Statement of Purpose for any registered services it provided in accordance with Care Quality Commission (Registration) Regulations 2009.

 

The Home First services were registered to provide time limited rehabilitation and reablement and provide support both within the residential setting and in people’s own homes.

 

Home First services were provided by a team of social and health care staff, who worked together to provide support and health therapies to achieve a person’s goals to become as independent as possible. 

 

The services work with individuals, and their circle of support, developing a programme to improve mobility, health needs, daily living activities and practical tasks.  This developed confidence, strength and skills for people to carry out activities independently, supporting them to remain living at home.

 

The services also worked in partnership with other health professionals to prevent early admission to hospital and facilitate early discharge.

 

The document referred to the Home First commitment to achieving improvements for the people of North Lincolnshire as set out in the council’s four strategic outcomes.  These were:

 

         Safe

         Well

         Prosperous

         Connected

 

Resolved – That the proposal to publish the Statements of Purpose for the Home First Services on the council’s website, be approved.
